1. General Disclaimer / He Whakamārama Whānui

MyLog is a digital care diary provided by MyMate Limited (NZBN: 9429052964442). It is designed to help families, whānau, and support workers record daily observations, preferences, and continuity notes for people with disabilities, chronic illness, dementia, or age-related care needs.

MyLog is a record-keeping tool only. It is not a medical system, clinical platform, legal instrument, or professional advisory service of any kind.

2. Not Professional Advice / Ehara i te Tohutohu Ngaio

Nothing on the MyLog platform, in its reports, exports, or on this website constitutes or should be relied upon as professional advice of any kind, including:

  • Medical or clinical advice, diagnosis, or treatment
  • Therapeutic or psychological advice
  • Legal advice
  • Financial or funding advice
  • Social work or disability support planning advice

Always consult a qualified professional before making decisions about health, care planning, medication, legal matters, or funding applications. MyLog records what you enter. It does not assess, interpret, or validate that information.

3. Accuracy of Information / Te Tika o ngā Kōrero

MyLog relies entirely on information entered by users. MyMate Limited makes no representation or warranty, express or implied, about the accuracy, completeness, reliability, currency, or fitness for purpose of any content recorded in or generated by MyLog.

Users are solely responsible for:

  • The accuracy of information they enter
  • Ensuring entries are timely and complete
  • Verifying information before relying on it for any purpose
  • Ensuring all authorised users understand their responsibilities

MyMate Limited does not monitor, moderate, verify, or validate any user-generated content.

4. PDF Reports and Funding Applications / Ngā Pūrongo PDF me ngā Tono Pūtea

MyLog’s export feature generates PDF reports based solely on information entered by users. These reports may be used to support funding applications, NASC assessments, ACC reviews, or other processes.

However, MyMate Limited does not:

  • Guarantee that any report will meet the specific requirements of any assessor, agency, or funding body
  • Guarantee that any report will result in a successful funding application or assessment outcome
  • Warrant that reports comply with the documentation standards of any particular organisation, including NASC, ACC, Whaikaha –  Ministry of Disabled People, or any support agency

You are responsible for verifying the requirements of any funding body or assessor directly before submitting a MyLog report. MyLog reports are a supporting tool, they do not replace formal care plans, clinical assessments, or documentation required by funders.

5. Emergency Situations / Ngā Āhuatanga Ohorere

⚠️ MyLog is NOT an emergency response system and is NOT monitored.
Never use MyLog to report a medical emergency, safety concern, or crisis situation.
For emergencies, always call 111 immediately.

The content published on this website (mylog.co.nz) is provided for general information purposes only. While we take care to keep information current and accurate, we make no guarantees about its completeness or accuracy at any given time. Content may be updated, changed, or removed without notice.

  • The service will be free from errors, bugs, or technical issues
  • Data will never be lost due to technical failure (users are encouraged to export records regularly as a precaution)
  • The service will meet every specific requirement of every user

Planned maintenance, outages, and improvements may cause temporary unavailability. We will provide reasonable notice where practicable.

9. Limitation of Liability / Te Aukatinga o te Takohanga

To the maximum extent permitted by New Zealand law, and subject to your rights under the Consumer Guarantees Act 1993 and other applicable consumer protection legislation, MyMate Limited shall not be liable for any:

  • Direct, indirect, incidental, or consequential loss or damage
  • Loss arising from reliance on information recorded in or generated by MyLog
  • Loss arising from a failed funding application or assessment outcome
  • Loss arising from service unavailability or data loss
  • Loss arising from actions or omissions of other users

Your statutory consumer rights under New Zealand law are not affected by this disclaimer. If you believe our service has failed to meet the guarantees required by law, please contact us at info@mylog.co.nz.

10. Governing Law / Te Ture Whakahaere

This Disclaimer is governed by and construed in accordance with the laws of New Zealand. Any disputes arising from this Disclaimer shall be subject to the exclusive jurisdiction of the courts of New Zealand.
